If you are a new patient to our office, the link beklow will allow you to complete new patient forms that will need to be filled out when you arrive at our office. Completing the forms online ahead of time will allow us to attend to your medical needs more quickly than completing them on your arrival.

Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance
If you have Insurance/dental benefits plan, we are committed to assisting you in getting your maximum of your benefits. Our experienced administrative team will assist you in preparing the necessary forms. As a courtesy, we will submit all of the claim forms to your dental plan company.
Keep in mind that we feel it is important that you know that dental plans can help with dental treatments however the patient is fully responsible for any amounts not covered/paid by their benefit plan.
